About You In Home Care is seeking a dedicated Licensed Practical Nurse (LPN) to oversee and support our team of CNAs and caregivers. The LPN Supervisor will report directly to the Clinical Director and play a leadership role in daily operations.
Responsibilities:
- Supervise and support caregivers - Provide training, mentorship, and oversight to ensure quality care and adherence to policies.
- Monitor patient health - Conduct regular assessments, document changes in health status, and update care plans as needed.
- Ensure compliance and accuracy in documentation - Maintain thorough records of patient care, interventions, and observations in accordance with state and company standards.
- Administer medications and treatments - Ensure safe administration of medications and treatments as prescribed.
- Act as a liaison - Communicate effectively with patients, families, caregivers, and healthcare providers to ensure continuity of care.
- Promote a positive work environment - Foster teamwork, encourage professional development, and address any concerns among staff.
Qualifications:
- Current LPN license in Mississippi
- Experience in home health, long-term care, or a supervisory role preferred
- Strong documentation, organizational, and leadership skills
- Ability to manage multiple responsibilities and make sound clinical decisions
- BLS certification required
