Position: Family Advocate
Location: LDP, 10058 South Mountain Road, South Mountain, PA 17268
The Family Advocate will help ensure there is a collaborative process with families, placing agencies and provider to have a specific focus on the best interest of each child. Planning and process will focus on aftercare/discharge planning and case management support services to each child.
Salary: $25.00 Hourly
Job Type: Full-time
Shift: Monday-Friday, 9am-5pm

Job Duties:
- Coordinates and implements case management activities with family / discharge planning
- Provides indirect supervision of clinical / case management positions to direct and monitor clinical services related to family and discharge planning
- Monitors level of family involvement/engagement and assists treatment team in making recommendation on client’s best interest related to family supports
- Assists in the development and implementation of individualized treatment plans
- Assists clients in the understanding of established treatment goals, service plans, and rights and expectations
- Assists clients in knowing what resources and supports are available to them at the program and in the community
- Provides or assists in group counseling sessions, facilitating and helping clients understand information being presented
- Participates in and documents services provided, as well as communication with external agencies/services involved in supporting client treatment plans
- Writes reports accurately and to support advocacy for client and family involvement
- Attends court hearings, advocacy meetings, and treatment planning meetings as necessary
- Communicates effectively with juvenile probation officers/case workers, including them in the treatment process and updating them on client progress related to responsibilities
- Maintains regular contact and communicates effectively with parents/legal guardians, including them in the treatment process and updating them on client progress
- Provides recommendations on continuing care planning by making contacts to appropriate referrals, communicating with the treatment team and creating the plan for reintegration
- Assists in services related to Specialized Treatment settings to ensure standards are met
- Supports the Abraxas philosophy and mission and promotes the Seven Key Principles of care.
- Demonstrates appropriate use of Safe Crisis Management techniques and skills.
Hiring Requirements:
The following education requirements are acceptable for this classification including:
- Bachelor’s Degree (preferably in a Human Services field of study)
- Minimum of two years’ experience in social services or related field
- Background / experience with trauma informed care, family dynamics, conflict resolution and cultural diversity is preferred
- Other Qualifications:
- At least twenty-one (21) years of age.
- Must pass both federal and state background checks which screen for crimes including, but not limited to drug related crimes, violent crimes, sexual abuse, domestic violence, child abuse, and repeated misdemeanors.
- Non-communicable diseases physical exam.
- Valid driver's license from employee’s state of residence.
- Valid registered vehicle insurance.
- Ability to work with computers and the necessary software typically used by the department
