Licensed Practical Nurse
SHIFT : Full-Time, Part-Time
Key Responsibilities :
- Provide direct patient care under the supervision of an RN and within the scope of LPN licensure.
- Administer medications, treatments, and wound care following physician orders.
- Monitor vital signs, patient progress, and report changes to the nursing team.
- Ensure compliance with infection control protocols, HIPAA regulations, and state / federal healthcare guidelines.
- Assist with ADLs (Activities of Daily Living) and provide compassionate resident care.
- Document patient care in electronic medical records (EHR / EMR systems).
- Collaborate with nurses, physicians, CNAs, and therapists to ensure optimal resident outcomes.
Qualifications & Requirements :
Active LPN License in the state of [State] (Required).
Experience in long-term care (LTC), skilled nursing (SNF), assisted living, or rehabilitation preferred.
Strong knowledge of medication administration, wound care, and infection control.
Excellent communication, teamwork, and patient care skills.
Ability to multitask in a fast-paced healthcare setting.
