What are the responsibilities and job description for the Lead Internal Medicine Physician position at Absolute Care?
AbsoluteCare is on the lookout for a passionate and mission-driven clinical leader to implement team-based collaborative care and population health for our medically complex and socially vulnerable members. We specialize in caring for individuals who have often been overlooked by the traditional healthcare system, leading to frequent visits to emergency rooms and hospitals. Our dedicated healthcare teams aim to transform the experience of our members by enhancing their quality of life, improving health outcomes, and reducing unnecessary healthcare costs. This exceptional care is delivered through an integrated team approach that includes primary care providers, complex care managers, nurses, medical assistants, behavioral health clinicians, and community health workers. We provide coordinated medical care in an environment equipped with an onsite infusion center, a full pharmacy, and lab and imaging services. Our commitment goes beyond clinical care to also address the social determinants of health, focusing on issues like housing instability, food insecurity, and social isolation.
The Lead Physician will play a crucial role in delivering direct clinical care to a panel of members while guiding fellow clinicians on best practices for effective value-based care. This role requires close collaboration with our Medical Director, as well as providing leadership and support to our team of providers, nurses, behavioral health specialists, and medical assistants.
Key responsibilities include :
- Delivering exceptional primary care to a panel of members and leading an interdisciplinary care team.
- Mentoring fellow providers on how to deliver outstanding primary care, urgent care, or community care while effectively managing their interdisciplinary care teams. Participate in population health huddles to help achieve quality and utilization goals.
- Acting as the clinical lead within the center alongside the Medical Director, addressing queries and providing guidance on clinical cases to nurses, medical assistants, and fellow providers as needed.
- Being a vital part of the center's and market's leadership team, responsible for steering the market toward achieving quality, utilization, and engagement goals. In collaboration with the Medical Director, participate in leadership meetings and serve as AbsoluteCare’s ambassador to community organizations, hospital systems, and payer partners.
