A Home Health Admissions Nurse is primarily responsible for performing the admission visit for home health patients referred to our service. The Admissions nurse must have a good understanding of OASIS, and have good assessment skills that enable them to recognize the need for other disciplines that may be necessary on a case and not yet ordered by the physician. This applicant must possess excellent communication and organizational skills. The duties of an RN as stated below also apply; however, the role of this nurse is to work to admit patients.
The Registered Nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on health education/experience. The nurse builds from the resources of the organization to plan and direct services to meet the needs of individuals and families within their home.
Patient Care:
- Completes an initial assessment of patient and family to determine home care needs.
- Provides a complete physical assessment and history of current and previous illness(es).
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan, which establishes goals, based on nursing diagnosis, and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan.
Additional Duties:
- Ensures that arrangements for equipment and other necessary items and services are available.
- Evaluate incoming patients to determine the severity of their illnesses and the intensity of services they will require. The admissions nurse will be the first caregiver that spends a significant amount of time with the patient, collecting medical-history information and discussing medication histories and a likely plan of care. The nurse will also give the patient information about what to expect during their home health experience.
Skills and Experience
A successful admission nurse will be skilled at "processing" patients - that is, working with one patient and then sending that patient off to another nurse's care. Admissions areas are often fast-paced and require good judgment to determine the initial course of the patient's care. Must be able to perform assessment/data collection in manner that includes mental and cognitive status, family/social /developmental issues and functional ability.
- One year of RN experience, Homecare experience preferred.
- Valid driver’s license and reliable transportation required.
- Computer proficiency required.
