Job Summary
AbsoluteCare is seeking an experienced Advance Practice Clinician (NP, PA) with a dynamic passion for clinical practice as well as the education and training of patient-facing staff members. The Advanced Practice Clinical Trainer (APCT) maintains a working relationship with all AbsoluteCare (ABC) centers to support their onboarding needs, maintain regular cadence of trainings, and facilitate organizational change management. The APCT is well versed in AbsoluteCare policies and procedures and works collaboratively with the Clinical Services team to standardize practice and performance standards and expectations. This position supports both Nursing and Provider roles across the enterprise to ensure clinicians at ABC are functioning to the tops of their licenses in a manner that preserves the safety, efficacy, and efficiency of patient care delivery.
The APCT is a split role with 20% being direct patient care (Avg. 1 day/week) and 80% dedicated to the development and implementation of training. Direct patient care can involve supporting the needs of the primary care team, and also the immediate care area for urgent patient needs. This role supports the enterprise, requiring local and national travel based on business needs in addition to virtual training support for staff working for a multi-state healthcare organization.
Duties and Responsibilities
Provider Practice:
- Provide primary care services to new and established members of the practice.
- Work in a team environment (with social work, care management, behavioral health, nursing, pharmacy, nutrition, etc.), process feedback, and facilitate discussion and implementing plans of care from other members of the team.
- Treatment of acute exacerbations of chronic conditions onsite.
- Possess a comfort level and the knowledge to differentiate when exacerbation can be treated within AbsoluteCare and when a higher level of care is needed.
- Accept same day/next day or urgent appointments for patients from home or discharged from the hospital/ER/skilled nursing facility in need of intensified physician or case management/social work services to ensure acute episode has resolved and the patient is not admitted/readmitted.
- Practice evidenced based medicine and in accordance with the PCMH model of care.
- Able to keep track of and successfully obtain specified quality metric goals.
- Perform other duties as required consistent with chronic care and disease management services provided to patients.
- Implement plans of care in collaboration with the patient, their family and other team members with strong transitional care plans and able to keep track of paneled patients along the whole continuum of care including hospital discharges, ER, skilled nursing facilities and home care.
- Other duties as assigned on site.
Clinical Education and Training:
- Work alongside the NHE (New Hire Experience) team, Medical Directors, and Directors of Center Operations to plan Week 3 Onboarding through transition to independent practice.
- Plan and prioritize needs for new center implementation alongside the clinical services team.
- Collaborates with Directors of Informatic and EHR Informatics Specialist to ensure most up-to-date eCW content is included in Nursing and Provider trainings, and updates associated job aids or references as needed.
- Serve in or assign preceptorship for new providers during orientation across the enterprise.
- Provides coaching and feedback to training participants and business units to ensure seamless transition into On-the-Job shadowing/preceptor phase and successful completion of training (parallel to existing Clinical Educator JD)
- Establish/maintain clear, measurable and comprehensive competencies for Nursing and Provider roles consistent with best practice standards, which APCT follows to completion.
- Collaborate with Clinical Services team on clinical policies and procedures as needed.
- Collaborate with Informatics team on clinical pathway implementation.
- Collaborate with the Clinical Applications Team to keep training materials updated, participate in eCW upgrades, identify areas of optimization, and troubleshoot eCW issues.
- Establish, organize, and execute quarterly training events across the enterprise encompassing essential Nursing and Provider skills needed for on-site practice.
- Collaborate with medical staff to improve patient care and safety.
- Participate in ongoing group teaching to clinical team members through virtual and in-person venues.
- Skillfully assemble and present content with Microsoft Office tools (e.g. PowerPoint, Excel).
- Meet regularly with the Senior Director of Clinical Services to streamline and integrate services across clinical disciplines.
- Other duties as they arise pertaining to education, training, instruction, and performance improvement efforts.
Minimum Qualifications
- Active, unrestricted medical license in the state of employment.
- 5 years of clinical experience and administrative experience leading interdisciplinary teams.
- 3-5 years of experience in an educator/trainer capacity.
- Experience working for a clinic, community-based organization & inpatient unit.
- Experience with training and mentoring.
- Excellent interpersonal communication skills.
- Strong public speaking skills.
- Strong problem solving, project management and organization skills.
- Independent self-starter and critical thinker interested in population health, healthcare utilization management, and the ongoing development of our care model.
Working conditions
This job operates in a professional office environment. This role routinely uses general office equipment. Often wear a special uniform, such as a lab coat or surgical scrubs, protective or safety attire, such as gloves and masks. Exposed to the diseases and infections of patients daily. May share workspace with other technicians and staff. Will be required to travel to any of our centers or corporate office based upon company needs.
Physical requirements
- Ability to communicate clearly and exchange accurate information constantly.
- Ability to travel regularly across the enterprise according to business needs up to 2 trips per month spanning 5 business days at a time (40 working hours) per week.
- Ability to remain stationary for long periods of time.

None at this time, though in the future this role could grow to include oversight of a team of providers and nurses who work on building and maintaining training guides, courses, and integrated clinical support tools.
