Job Description:

Job Summary

This role supports the Community Based Nurse Practitioner (CB-NP) as they care for patients in their homes and in the community. The CB-NP and Community MA are members of our Integrated Care Team. Together, they perform hospital follow-up visits for patients at risk for readmission with emphasis on ensuring a smooth transition. This includes evaluating and updating the discharge plan, completing medication reconciliation, and providing patient, family, and caregiver education. The CB-NP may also carry a panel of patients, performing primary care for those who are unable to come into our medical center. Visit support by the Community MA includes measuring vital signs, administering medication, phlebotomy, scheduling, and administrative care coordination.

Duties and Responsibilities

  • Act as the community-based medical assistant, supporting community-based providers in facilitating high quality primary, transitional, and acute care that promotes appropriate healthcare utilization.
  • Drive to and from the medical center and member home/community.
  • Manage CB-NP schedule and schedule follow up appointments.
  • Prepare members for in person and telehealth examination. Take vitals, perform appropriate social, behavioral, and medical age and condition-based screenings.
  • Close quality measure gaps.
  • Maintain medical record completion. Document key aspects of medical and social history. Initiate medication reconciliation.
  • Assist the provider with exams and minor procedures.
  • Perform phlebotomy and collection of other lab specimens.
  • Prepare and administer medications and vaccines with provider authorization.
  • Review the after-visit summary with the member and ensure understanding through communication tools like teach-back and closing the loop.
  • Ensure follow-up visits are scheduled prior to visit completion.
  • Follow protocols for infection control, including proper cleaning and disinfection of medical equipment.
  • Perform pre-visit planning to ensure high quality provider visits.
  • Coordinate services to complete community-based provider action plan tasks.
  • Provide coverage support for fellow care team members onsite as needed.
  • Leverage strong time management skills to prioritize and balance member care responsibilities with role-specific tasks.

Minimum Qualifications

  • Certified or Registered Medical Assistant in applicable state – OR – a valid NREMT-A or Paramedic certification with current state certification.
    • For MA candidates, 3 years of experience practicing as a Medical Assistant required.
    • For EMT candidates, minimum of one year of experience in the pre-hospital setting required.
  • High School Diploma or equivalent required.
  • Active CPR Certification.
  • Phlebotomy skills.
  • Able to react quickly in emergencies and identify appropriate staff during crises.
  • Able to efficiently prioritize multiple high-priority tasks.
  • Proficiency with Microsoft Office applications.
  • Ability to operate personal motor vehicle preferred.
  • Active driver’s license and proof of insurance in state of practice preferred.

Working conditions

This job operates in the community and within a professional office environment. Community settings can vary and include patient homes, homeless shelters, and shared public spaces. This role routinely uses general office equipment and often wear a special uniform, such as a lab coat or surgical scrubs, protective or safety attire, such as gloves and masks. Exposed to the diseases and infections of patients daily. May share workspace with other technicians and staff.

  • Ability to remain stationary for long periods of time.
  • Constantly operates computer, keyboard, copy and fax machine, phone, and other general office equipment.
  • Operates personal motor vehicle.
  • Ability to occasionally move objects up to 20 lbs.
