Position Overview:
We are seeking a highly skilled and experienced Licensed Practical/ Licensed Vocational Nurse (LPN/LVN) to join our team. The ideal candidate will have a strong background in providing specialized nursing care, including tracheostomy care, ventilator management, IV therapy, wound care, and other advanced nursing interventions. About the Role:
As a LPN/LVN, you will deliver skilled nursing care to adult patients with complex medical needs, focusing on both short-term recovery and long-term management of injuries or conditions.
Patients You Will Care For:
- Adults with chronic illnesses or post-hospitalization recovery needs including:
- Traumatic brain injuries (TBI)
- Spinal cord injuries
- Severe burns
- Orthopedic injuries (fractures, post-surgical care)
- Amputations
- Neurological disorders related to workplace accidents
- Long-term disability cases requiring skilled nursing interventions
Key Responsibilities:
- Provide high-tech skilled nursing care in patients' homes, including tracheostomy and ventilator care, central line management, wound care, and IV infusion therapy.
- Administer medications, monitor patient progress, and adjust treatments as needed.
- Educate patients and families on disease management, equipment usage, and self-care techniques.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care.
- Maintain accurate and timely documentation in accordance with state and agency regulations.
- Ensure compliance with all federal, state, and local healthcare laws and regulations.
Qualifications & Requirements:
- Active Licensed Practical/ Licensed Vocational Nurse (LPN/LVN) license in Illinois (required).
- Minimum 4 years of experience in hi-tech nursing, home healthcare, or an acute care setting. (preferred)
- Expertise in trach/ventilator care, IV therapy, wound care, and complex medical case management.
- Certification in ACLS/BLS (required); additional certifications in home health or critical care nursing preferred.
- Strong critical thinking, problem-solving, and decision-making skills.
- Ability to work independently and efficiently in a home healthcare environment.
- Excellent communication and interpersonal skills to work effectively with patients, families, and medical teams.
- Reliable transportation and willingness to travel within Cook, Lake, and DuPage Counties.
