Position Overview: We are seeking a compassionate and dedicated Family Nurse Practitioner (FNP) to join our team. The ideal candidate will provide comprehensive healthcare services to both pediatric and adult patients with holistic and integrative approaches. This role emphasizes preventive care, patient education, and the management of both acute and chronic conditions within a family-centered model.
Key Responsibilities:
Patient Care:
- Conduct thorough health assessments, including patient history and physical examinations.
- Diagnose and manage acute and chronic conditions for both pediatric and adult patients.
- Develop and implement individualized treatment plans, incorporating both integrative and holistic modalities.
- Prescribe medications and natural remedies as appropriate, ensuring compliance with state and federal regulations.
- Provide patient and family education on disease prevention, nutrition, and healthy lifestyle choices.
Collaboration:
- Work closely with a multidisciplinary team, including other providers, administrative staff, support staff, laboratory facilities
- Participate in case discussions and contribute to the development of comprehensive care plans.
Documentation:
- Maintain accurate and timely electronic health records (EHR) of patient interactions, treatments, and progress.
- Ensure documentation meets all regulatory and compliance standards.
Professional Development:
- Stay current with advancements in both conventional and holistic care.
- Participate in continuing education opportunities and professional organizations relevant to family practice and holistic medicine.
Qualifications:
Education & Certification:
- Master’s or Doctoral degree in Nursing from an accredited institution.
- Current unencumbered licensure as a Family Nurse Practitioner in the state of [PA].
- National Certification through the American Nurses Credentialing Center (ANCC), the American Academy of Nurse Practitioners (AANP), or other accrediting body.
Experience:
- Minimum of [1] years of experience in family practice settings Preferred
