What are the responsibilities and job description for the Licensed Practical Nurse-Wound Care position at Acadian Medical Center?
1. Carries out hospital / nursing policies, procedures, and standards.
- Assess the unique needs of each patient to determine appropriate treatment plans. Perform initial nursing assessment, communicate findings with assigned physician. Assess the wound stage and obtain measurements. Document care plans, evaluate the patient' status in regard to desired outcomes, and identify patient care situations that require intervention.
- Application of topical wound care and associated wound management products.
- Work collaboratively with other members of the patient care team to determine if additional treatments or changes in care are required.
- Recommendation of associated services to improve wound healing outcomes. This includes, but is not limited to, labs, radiologic tests, vascular tests, referrals, home health care, Vacuum Assisted Closure device, and orthotics.
- Recommendations for prescription of required medications and / or Durable Medical Equipment (DME) to facilitate wound healing and / or wound prevention within her / his scope of practice, license and as per the collaborative agreement with the supervising physician.
- Educate patients and caretakers on wound care, infection and injury prevention, pressure ulcer (bed sore) care, and other best practices to promote healing and wellness
- Coordinate with physician to expedite wound healing and avoid skin breakdown.
- Track treatments and patient progress according to HIPAA guidelines.
10. Provide patient updates to PCP, CM, and Physicians.
