Job Summary
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. This role allows the practitioner to see patients in the place they call home and offer traditional & attentive patient care experiences. The ideal candidate will provide high-quality patient care, focusing on symptom management, diagnosis, and treatment of various medical conditions. This role requires a strong clinical background and the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes.
Responsibilities
- Conduct comprehensive in-home physical examinations and health assessments for patients.
- Diagnose and manage acute and chronic illnesses, including developing treatment plans.
- Document patient medical histories and maintain accurate medical records in compliance with legal and ethical standards.
- Collaborate with physicians and other healthcare providers to coordinate patient care and referrals.
- Provide education to patients and families regarding health management, disease prevention, and wellness strategies.
Requirements
- Master’s degree in Nursing or related field from an accredited program.
- Current Nurse Practitioner license in the state of Texas.
- Strong clinical skills with experience in acute care settings preferred.
- Proficiency in medical documentation practices.
- Knowledge of occupational health principles is a plus.
- Experience with infant care is desirable but not mandatory.
- Familiarity with sterile processing techniques is advantageous.
- Ability to conduct utilization reviews effectively.
- Excellent communication skills and a compassionate approach to patient care.
