The Patient Assessment Coordinator (PAC) coordinates the completion and transmission of the Inpatient Rehab Facility Patient Assessment Instrument (IRF-PAI) within appropriate time frames in accordance with Centers for Medicare and Medicaid Services. Specifically, the PAC oversees all aspects of data collection on the IRF-PAI, ensures the presence of supporting documentation, and provides staff education to ensure compliance with internal policy and external regulatory requirements, including applicable Center for Improvement in Healthcare Quality (CIHQ) standards, and CMS guidelines. In addition, the PAC develops and interprets policies and procedures and acts as the primary resource for immediate problem solving regarding IRF-PAI scoring. This position requires a positive working relationship with interdisciplinary teams.

MINIMUM QUALIFICATIONS :
- Be a graduate of an accredited Registered Nurse (RN), Occupational Therapy (OT), Physical Therapy (PT) or Speech Language Pathology (SLP) program with current Texas licensure
- Two years clinical experience in the care of patients in an acute hospital, rehabilitation or LTACH setting required
- Experience in IRF PAI or MDS coordination (preferred)
- Must have leadership ability and good communication and interpersonal skills
- Working knowledge of Medicare guidelines for Rehabilitation (acute rehab criteria, level of care requirements and medical necessity)
- Ability to function within an interdisciplinary environment
- Ability to read and speak English clearly and be understood
- Understand Medical terminology, regulations and parameters of third party reimbursements, and coding guidelines
- Have computer knowledge including Outlook, Excel, Word, PowerPoint, and data management software required
