Access Behavioral Health Services is looking for a passionate Case Manager to join our Assertive Community Treatment (ACT) Team that provides expert care to individuals with serious and persistent mental illness (SPMI) right in the heart of the community!

At Access Behavioral Health, we believe in meeting people where they are – not just staying within the confines of 1 service that is limited. Whether you're in the field or in the office, you'll be providing care much like you have but you’ll have the time to spend with each client that demonstrates the genuineness of your care. Our clients face complex challenges, and you’ll be part of a team-based approach that assists them over the roadblocks which makes recovery possible without the hospital re-admissions.

Position: Case Manager (CM)

Location: Boise, ID (Community-based & In-office)
Pay: $50,000-60,000 annually DOE
Schedule: Full-time with paid trainings (LIKE TONS OF TRAINING). Flexible hours that can be scheduled to meet needs, but 40 hours per week is expected relatively consistently.

What You’ll Do:

  • Community-Based Care: Provide vital Case Management services to individuals in the ACT Team who are dealing with serious and persistent mental illness (SPMI). You'll get out of the office and directly engage with clients where they live, work, and interact.
  • Collaborate with the Team: Work with an interdisciplinary team of professionals (including social workers, therapists, and case managers) to develop and implement care plans, manage medications, and provide essential mental health support.
  • Case Management: Assess, educate, and manage resources and referrals for individuals, ensuring access to resources and care monitoring and coordination with participant providers and supports.
  • Crisis Support: Assist with crisis interventions, provide risk assessments, and help client’s access services and resources if needed.
  • Client Education: Provide education to clients and staff on resources, resource utilization, mental health, and wellness in both group and one-on-one settings.
  • Comprehensive Support: Help clients develop healthy coping mechanisms, navigate resources, and foster a strong support network, while tracking progress and adjusting care plans as needed.
  • Community Engagement: Represent Access Behavioral Health in a positive and professional manner within the community. You’re not just providing care—you’re building relationships and making a lasting impact.
  • On-call Support: Be part of a humane on-call system to support the ACT Team in the field when needed.

What We Need from You:

  • Bachelor’s Degree in a human service field: Must have a 4 year degree from an accredited university
  • experience with the behavioral health population is stronglypreferred.
  • Reliable Transportation: You’ll need a valid driver’s license, insurance, and a vehicle (or access to a company vehicle).
  • Strong Communication Skills: You’ll need to engage with a diverse range of clients, family members, and professionals, and be able to communicate clearly and empathetically.
  • Ability to Learn: Knowledge of community-based interventions, and mental health service delivery models is helpful, or be willing to learn quickly.
  • Team Player: Must be able to work well within a multidisciplinary team and collaborate with colleagues to provide comprehensive care.
  • Crisis Intervention: Ability to perform risk assessments, develop safety plans, and provide crisis support when necessary.Why This Job is Different:
