What You'll Do:
- Support individuals in achieving their personal goals through personalized support plans.
- Provide transportation to assist individuals in attending appointments, errands, and community outings.
- Assist individuals with daily living activities, promoting independence and dignity.
- Encourage individuals to participate actively in their communities, helping them discover hobbies and build meaningful relationships.
- Lead physically engaging activities such as walking, hiking, and participating in community events.
- Maintain accurate and timely documentation, including daily logs, progress notes, and incident reports.
- Assist individuals in maintaining a clean, safe, and orderly home environment by educating them on safety and hygiene practices.
- Support individuals with fiscal management tasks such as budgeting, bill payments, and balancing checkbooks.
- Ensure individuals exercise their rights and are treated with dignity and respect.
- Maintain and enhance skills through ongoing required training and certifications.
Work Location & Schedule:
- Yamhill and surrounding areas based on individual and program needs
- Part-Time positions (minimum of 20 hours per week)
- Full-Time positions (minimum of 30 hours per week)
Qualifications:
- Must be 18 years of age or older with a valid driver’s license
- High school diploma or equivalent
- Reliable transportation and willingness to use a personal vehicle for client transportation
- Clean driving record
- Ability to pass a background check
- Strong verbal and written communication skills; basic computer proficiency
- Genuine desire to help others achieve their goals and positively impact their lives
- Ability to maintain flexibility with work schedules and adapt to changing conditions
