This role is essential in chronic disease management, diabetes education, and preventative health, empowering patients with the knowledge and tools needed to make informed dietary choices that support long-term wellness. The role involves direct patient care, program management, research participation, and interdisciplinary collaboration to improve overall health outcomes and enhance the quality of life for patients.

The pay ranges provided represent the minimum to mid-range for positions. Actual compensation will be determined based on a combination of factors including years of experience, educational background, market conditions, and available grant funding.

Core Job Responsibilities
- Educate patients and families on disease prevention, chronic disease management, and nutrition-related disorders in an outpatient setting.
- Assess patients’ nutritional needs, create tailored meal plans, and coordinate care with physicians, diabetes educators, and healthcare teams.
- Ensure the program meets the American Diabetes Association (ADA) recognition standards, with accurate documentation and compliance.
- Perform other responsibilities as assigned.
- Bachelor’s degree required. Master of Science in Dietetics or Nutrition strongly preferred.
- Valid Illinois license as a Registered Dietitian required.
- Certificate Diabetes Educator (CDE) preferred.
- Minimum of two (2) years’ experience as a certified diabetes educator preferred.
- Experience in an outpatient diabetes center strongly preferred.
- Experience with American Diabetes Association (ADA) recognition standards strongly preferred.
- Bilingual (English/Spanish) preferred.
- Proficiency in Electronic Health Record (EHR) and EPIC Preferred
- Intermediate proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
- Normal office and health center environment regularly
- Local travel between health centers and community events is required; mode of transportation required.
- If a personal vehicle is utilized, a current driver’s license and proof of insurance are required.
