Licensed Practical Nurse (LPN) β Home Health Hospice
Job Type: Part-Time/Per Diem
Location: Las Vegas, Henderson Area
Reports To: Registered Nurse (RN) Case Manager / Clinical Supervisor
Job Summary:
The Licensed Practical Nurse (LPN) in a home health hospice setting provides compassionate, high-quality nursing care to terminally ill patients in their homes or residential facilities. Under the supervision of an RN, the LPN ensures comfort, dignity, and symptom management while supporting the patient and their family during end-of-life care.
Key Responsibilities: Patient Care & Clinical Duties:
- Provide direct nursing care following the hospice plan of care and under the supervision of an RN.
- Administer medications, treatments, and pain management interventions as prescribed.
- Monitor and document patient symptoms, vital signs, and overall condition.
- Assist with personal care, including bathing, dressing, and hygiene, as needed.
- Educate patients and families on disease progression, symptom management, and hospice philosophy.
- Recognize changes in patient condition and report them promptly to the RN or physician.
Coordination & Communication:
- Collaborate with the interdisciplinary hospice team, including RNs, physicians, social workers, and chaplains.
- Maintain accurate and timely documentation in the electronic medical record system.
- Provide emotional support and compassionate care to patients and families.
- Participate in team meetings, case conferences, and ongoing education.
Compliance & Safety:
- Follow policies, procedures, and regulatory guidelines.
- Adhere to infection control and safety protocols.
- Ensure compliance with state and federal hospice regulations.
Qualifications:
- Licensure: Active LPN license in the state of Nevada.
- Experience: Previous hospice and/ or home health experience required.
- Certifications: CPR certification required. IV certification preferred
- Skills: Strong clinical assessment skills, compassionate communication, and ability to work independently in home settings.
Work Environment & Physical Requirements:
- Requires travel to patients' homes; reliable transportation is necessary.
- May involve lifting, bending, and extended periods of standing or walking.
- Exposure to various home environments and conditions.
Salary : $60 - $65
