Job Summary:
We are seeking a highly skilled and licensed VA Pharmacist to join our team. The ideal candidate is a healthcare professional specializing in the safe and effective use, storage, preservation, and distribution of medications. This role involves working closely with healthcare teams to ensure optimal patient care, medication safety, and compliance with federal regulations.
Key Responsibilities:
- Review and interpret prescriptions to ensure accuracy, safety, and proper dosage.
- Counsel patients on medication use, potential side effects, and storage requirements.
- Collaborate with healthcare providers to develop and adjust patient medication plans.
- Monitor and manage medication therapy for effectiveness and adverse reactions.
- Ensure compliance with VA policies, state and federal regulations, and professional standards.
- Maintain accurate patient records and documentation.
- Provide guidance on drug interactions, contraindications, and proper administration.
Minimum Qualifications:
- Licensure: Must hold an active pharmacy license in a U.S. state, the District of Columbia, the Commonwealth of Puerto Rico, or a U.S. territory.
- Education: Must have a Doctor of Pharmacy (Pharm.D.) degree from a program accredited by the Accreditation Council for Pharmacy Education (ACPE) or another accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
- Experience: At least two (2) years of clinical staff pharmacist experience is required.
- Strong knowledge of medication safety, storage, and best practices in pharmaceutical care.
- Excellent communication and collaboration skills to work effectively with patients and healthcare teams.
Preferred Qualifications:
- Experience in a VA or government healthcare system.
- Board certification or specialized training in clinical pharmacy practice.
- Familiarity with electronic health records (EHR) systems used in VA facilities.
This position is contingent upon contract award, client approval, successful background investigation, and meeting credentialing requirements.
