At Access Hospice, we are dedicated to delivering exceptional end-of-life care to our patients and their families. As a Hospice RN Case Manager, you will play a vital role in ensuring that our patients receive the highest quality of care, tailored to their unique needs.
Responsibilities
- Makes the initial nursing evaluation within forty-eight hours of referral to determine eligibility for hospice services.
- Identifies and re-assesses patients' physical, psychosocial, and environmental needs every fifteen days.
- Initiates and coordinates the plan of care, maintaining accurate clinical notes on patient condition.
- Documents problems, goals, interventions, and patient response to hospice care.
- Collaborates with attending physicians, patients, and families in providing daily care.
Requirements
- One year of clinical experience as an RN.
- Current RN licensure in the state of practice and one year of clinical experience.
- Current CPR Certification and access to dependable transportation or public transportation.
