About the Role
The RN Case Manager will assume full nursing responsibility for delivering high-quality care to each patient, while continuously evaluating personal and professional performance to improve productivity and quality of care.
Key Responsibilities
- Makes the initial nursing assessment within forty-eight hours of referral.
- Develops and implements individualized plans of care based on patient needs.
- Collaborates with healthcare teams to provide seamless care coordination.
- Ensures timely and accurate documentation of patient information.
- Participates in ongoing education and training to enhance professional skills.
Qualifications
- Bachelor's degree in Nursing (BSN) or related field.
- Current RN licensure in good standing.
- Minimum one year of experience as an RN case manager or similar role.
