OVERVIEW OF PRIMARY RESPONSIBILITES:
This position will utilize the evidence-based model of Individual Placement and Support (IPS) Employment Services to assist adults with mental health conditions in obtaining employment through the Assertive Community Treatment (ACT) team. Additionally, this role includes providing clinical treatment and intensive, highly coordinated services to individuals with acute behavioral health needs. The ACT team delivers community-based, person-centered, trauma-informed, and culturally humble interventions to support individuals in maintaining stability in their home, school, and community environments.
PRIMARY FUNCTIONS:
- Job development and rapid job search for adults with mental health conditions.
- Job coaching, benefits counseling, and follow-along supports/ongoing job support.
- Conduct assessments and provide individualized clinical treatment, including crisis intervention.
- Collaborate with ACT team Vocational Specialists and other service providers.
- Participate in staff meetings, clinical case conferences, and interdisciplinary team discussions.
- Provide support and therapeutic interventions to youth and their families in home and community-based settings.
- Maintain accurate and timely documentation in electronic health records and reporting systems.
- Participate in a weekly on-call rotation.
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:
- Complete monthly data reports and enter data into NYESS and other reporting systems.
- Attend required trainings (IPS, CORE, ACT, NYESS, and agency-specific trainings).
- Maintain documentation for Ticket to Work and other employment support programs.
- Adapt to assignments as determined by program needs
QUALIFICATIONS AND ATTRIBUTES:
- Strong organizational skills with attention to detail.
- Excellent communication skills and ability to multi-task in a fast-paced environment.
- Ability to manage multiple simultaneous work demands effectively and professionally.
- Self-motivated, adaptable, and able to work both independently and collaboratively.
- Proficiency in navigating electronic health records and reporting systems.
- Bilingual in English and Spanish preferred but not required.
EDUCATION AND EXPERIENCE:
- License as LMSW/LMHC/LMFT/LCSW.
- At least one year of vocational rehabilitation experience required.
- Experience working with youth and families in both individual and group settings.
- Experience in community-based settings preferred.
