What are the responsibilities and job description for the Care Coordinator position at Access: Supports For Living?
We are seeking a skilled Care Coordinator to join our team at Access: Supports For Living. In this role, you will provide skill building training and supports to individuals with developmental disabilities in the community or their homes.
Responsibilities include:
- Providing mentoring/teaching services to help individuals achieve personal outcomes and growth
- Teaching daily living skills such as money handling, socialization, cooking, and job search
- Lifting or assisting with hygiene functions for selected individuals
- Working on valued outcomes with steps to achieve them based on caregivers/community supports and behaviors observed
Our ideal candidate has prior experience working with related populations, strong interpersonal and engagement skills, and ability to document services and understand regulations.
Bilingual English and Spanish speaking is a plus, but not required.
