Direct Support Professional 2 - Home and Community Care Specialist

Salary: $17.50 per hour, paid bi-weekly

Job Description:

Key Responsibilities:

• Advocacy: Assist individuals in achieving their desired personal outcomes by providing support and guidance.
• Health Maintenance: Ensure individuals receive optimal health care, including medical appointments, medication administration, and healthy meal planning.
• Safety: Provide continuous care to ensure the overall safety of individuals, colleagues, and self.
• Household Maintenance: Perform routine household tasks, such as cleaning, laundry, and meal preparation.
• Personal Care: Assist individuals with personal care needs, including showering, feeding, and physical assistance.
• Independent Living Skills Development: Help individuals develop skills to increase independence, such as money management and community integration.
• Mentorship: Serve as a role model, demonstrating appropriate social interactions and implementing approved crisis intervention techniques.
• Operational Support: Complete documentation, safety checks, and other tasks as required.
• Transportation: Provide transportation for individuals to various locations using agency vehicles.
