Enhanced Community Habilitation Worker - Columbia & Greene County
We are seeking skilled professionals to join our team as Community Habilitation Workers in the Hudson Valley area. Our mission is to empower individuals with developmental disabilities to live their healthiest and fullest lives possible.
Job Summary:
As a Community Habilitation Worker, you will provide skill-building training and support to individuals with developmental disabilities in their homes or communities. Your expertise will help them achieve personal outcomes and growth by teaching daily living skills, socialization, cooking, job search, and other essential skills identified in their individual plans.
Key Responsibilities:
• Provide mentoring and teaching services to individuals with developmental disabilities to help them achieve personal outcomes and growth.
• Teach daily living skills such as money handling, socialization, cooking, job search, and other essential skills identified in the individual's plan.
• Assist individuals with lifting or hygiene functions as needed.
• Collaborate with caregivers and community supports to develop personalized plans for each individual.
• Meet with assigned individuals several times per week, depending on coordinated hours.
• Travel to assigned locations to provide services.
• Attend required trainings to stay up-to-date on best practices and regulations.
• Document training sessions and report progress and barriers to supervisors.
Qualifications:
We seek candidates with:
• Prior experience working with related populations, including intellectual developmental disabilities and autism spectrum disorders.
• Strong interpersonal and engagement skills.
• Ability to document services and understand regulations.
• Bilingual English and Spanish speaking a plus.
