What are the responsibilities and job description for the Transition Specialist position at Access-Supports for Living?
Location: Middletown, NY
Hours: 20 hours- Part Time (M-F 1 pm to 5 pm with some flexibility)
Rate of Pay: $16.02
Job Description:
In this position, the ideal candidate will work with at-risk families to provide Care Coordination utilizing the High Fidelity Wrap Model. This will enable families to identify and plan for their needs and the needs of the children in their care. The ultimate goal is to have families remain intact with all members continuing to live together successfully and fully integrated into their communities.
Qualifications:
- Minimum 1 year experience working with at-risk families and youth diagnosed with mental health
- MUST be flexible with work schedule (days, late afternoons, evenings, maybe an occasional weekend)
- Must have a valid & insurable driver's license
- Must have a reliable working vehicle, which will be used every day
Education:
Associate's Degree in any human services field
Bilingual- Spanish/English a plus
