What are the responsibilities and job description for the CCT/CCA Care Coordinator position at Access TLC Health Care?
Access TLC is seeking a bilingual Spanish-speaking, full-time Care Coordinator to join our team. The Care Coordinator will provide in-office case management and conduct regular field visits to patients. The ideal candidate will be responsible for coordinating care, providing support to Spanish-speaking patients, and ensuring they receive the necessary services in a timely and efficient manner. Fluency in both Spanish and English is required to effectively communicate with our diverse patient population.
Access TLC support patients by arranging for community-based and other services that allow recipients to experience an enhanced and enriched quality of life. The right candidate will be eager to learn, compassionate, kind and a genuine team player.
Position Responsibilities:
- Conducting assessments, developing care plans, arranging for and monitoring service delivery, maintaining progress notes and case records, conducting quality assurance reviews, and collecting data.
- Makes visits to patients to establish ongoing eligibility for the program, to monitor the safety and services identified in the Individual Service Plan and address the concerns of the patient.
Requirements Include:
- Must have either a Bachelor or Masters Degree in Social Work, Psychology, Counseling, Rehabilitation, Gerontology, or Sociology.
- Must have a clean California Driver’s License with a vehicle that is insured in accordance with state/or organization requirements
- One year of related work experience. Intern or volunteer experience is also applicable.
- Bi-Lingual in Spanish
