Position Overview : Access to Healthcare Network (AHN) is hiring a Health Resource Coordinator (Navigator) for the NVCC department. This position supports seniors and individuals with disabilities by connecting them with community resources and services. This role involves conducting assessments, assisting with program applications, and providing follow-up services. The position requires regular travel within rural Nevada, outreach efforts, and strong collaboration with local partners. Only Elko residents should apply.
Key Responsibilities :
- Provide long-term care options and service referrals to clients.
- Conduct assessments and help clients navigate available programs and services.
- Focus on seniors and individuals with disabilities.
- Partner with local agencies and conduct outreach in rural Nevada counties.
- Represent AHN at events, provide home visits, and follow up to ensure successful outcomes.
- Communicate effectively and comply with HIPAA regulations.
