Health Services Support Case Manager - Located Elko, NV
Job Purpose : Provide long-term case management and services assistance to seniors, individuals with disabilities, and caregivers. Requires extensive travel within Nevada counties and flexibility in work schedule. A company car is provided.
Duties / Responsibilities :
- Client Support : Conduct assessments, home visits, and establish case management plans. Provide one-on-one, phone, and email support.
- Documentation and Reporting : Maintain detailed case notes and provide monthly follow-ups.
- Advocacy and Education : Inform and educate consumers about long-term care options and assist with applications.
- Partnership Development : Collaborate with public and private partners, participate in outreach activities, and attend community meetings.
