SCHEDULE:
Full-Time, Monday – Friday, 8:00 AM – 5:00 PM;
Rotating Saturdays (8a – 12p) / Weekend availability
PAY RANGE:
$16.00 - $19.00

Job Summary
The Reentry Community Health Worker (CHW) will play a critical role in supporting individuals incarcerated and transitioning back into the community. This position aligns with the HHS initiative to strengthen healthcare access and improve transitions in care for people leaving incarceration. The CHW will assist individuals housed at the Fort Bend County Jail in maintaining healthcare services during and after transition, navigate healthcare and social services, and support their reintegration into the community by linking them with necessary resources. CHW will act as a liaison between healthcare providers, social services, and justice-involved individuals, ensuring continuity of care during reentry.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
- Screening and Referral Management:
- Screen and evaluate referrals from the Fort Bend County Jail, healthcare providers, and community partners to identify appropriate programs.
- Facilitate access to healthcare services (primary care, mental health, substance use treatment, chronic disease management) and ensure clients are linked to transition support (housing, food, transportation, employment).
- Client Engagement and Support:
- Assess the healthcare and social service needs of each client and complete intake assessments to develop individualized care plans.
- Provide case management services, including coordinating and scheduling appointments, assisting with transportation to healthcare visits, and addressing barriers to accessing care.
- Support clients in applying for Medicaid, CHIP, CHIP Perinatal, food assistance (SNAP), Temporary Assistance to Needy Families (TANF), and other relevant services.
- Facilitate communication between clients, healthcare providers, and community resources, acting as a trusted advocate for clients' needs.
- Documentation and Reporting:
- Document assessments, referrals, and follow-up activities in a timely manner.
- Prepare and submit weekly reports, including client progress updates, to management and program staff.
- Maintain and update program rosters to track client status and service engagement.
- Eligibility Determination and Maintenance:
- Monitor clients’ eligibility for healthcare services, ensuring continuous coverage and assisting with re-enrollment into programs such as Medicaid or CHIP as needed.
- Training and Professional Development:
- Participate in regular training to stay informed on changes to healthcare policies, reentry services, and the specific needs of justice-involved individuals.
- Compliance:
- Adhere to all company policies, regulatory requirements, and ethical guidelines.
- Ensure that all services and activities are following federal and state laws and healthcare standards.
- Other Duties:
- Perform any other duties assigned by management.
SKILLS AND EXPERIENCE:
- Two years of experience in customer service, care coordination, or case management.
- Experience documenting and navigating an Electronic Health Record (EHR) system.
- Ability to operate a computer, fax machine, and scanner.
- Good communication skills.
- Ability to work with people of all socioeconomic levels.
- Ability to communicate orally and in writing with good organizational skills.
- Be a consumer advocate for quality care and services.
- Reliable transportation required and experience/ comfort with home visits preferred.
Qualifications
EDUCATION/LICENSURE/CERTIFICATION QUALIFICATIONS
- High School Diploma or GED.
- Certified Community Health Worker or two years working in a community health center.
- Other experience may be accepted in lieu of Education requirements at the discretion of management.
PREFERRED:
1. Experience with Medicaid, Medicare, and the Healthcare Marketplace and CHIP insurance programs.
2. Knowledge of Fort Bend community resources.
3. Bilingual ability preferred.
4. Lived or peer experience working with reentry populations.
