Job Description:
We are seeking a compassionate and skilled LPN to join our home health care team. As a Home Health LPN, you will be responsible for delivering high-quality care to patients in their homes, ensuring they receive the best possible treatment while promoting comfort and recovery. Responsibilities:
- Provide direct nursing care to patients in their homes, including assessments, care planning, and nursing interventions.
- Monitor patient health status and progress, updating care plans as needed.
- Administer medications, wound care, intravenous (IV) therapy and treatments as prescribed by physicians.
- Educate patients and families on health conditions, self-care techniques, and treatment plans.
- Collaborate with interdisciplinary team members to ensure comprehensive care.
- Maintain accurate and up-to-date patient records in compliance with company policies and healthcare regulations.
- Promote patient safety, comfort, and overall well-being during visits.
Requirements:
- Current and valid LPN license in the state of MA.
- Minimum of 1 year of nursing experience (Home Health experience preferred).
- Strong assessment, clinical, and critical thinking skills.
- Ability to work independently and manage time effectively.
- Excellent communication and interpersonal skills.
- Valid driver's license and reliable transportation.
