As a key member of our healthcare team, you will provide expert cardiovascular care, focusing on prevention, diagnosis, and management of cardiac conditions in a value-based model.
Key Responsibilities:
- Provide expert cardiovascular care to individuals, focusing on prevention, diagnosis, and management of cardiac conditions in a value-based model.
- Collaborate closely with interdisciplinary teams, including primary care physicians, advanced practice providers, nurses, and other specialists to ensure comprehensive patient care.
- Conduct comprehensive cardiovascular assessments and diagnostic evaluations for patients within our internal referral network of over 80 Primary Care Providers.
- Educate patients and their families about cardiovascular health, disease prevention, and management strategies.
