What are the responsibilities and job description for the Hospice RN PRN - On Call Rotation position at Accord Hospice of Manhattan?
Accord Hospice of Manhattan is currently hiring for an On Call Nurse to work weekends and or weekday nights. Position is open for a flexibly schedule and many different options available for hours (inquire within!).
The On-Call Nurse is the after hours equivalent of a Primary Nurse. He or she is responsible for responding to all of Hospice patients and their family members, as well as handling referrals and community inquiries, when the office is closed (nights, weekends, and holidays).
The On-Call Nurse provides:
- Urgent clinical and psychosocial interventions for patients and their families.
- Skilled pain control and symptom management.
Collaboration with providers within and outside the agency to maximize patient outcomes. The work provides considerable latitude for independent judgment. The On-Call Nurse consults with the Primary Nurse, the Director of Patient Services, Associate Director of Patient Services and/or physicians as necessary. General direction is provided by the Director the Medical Director. The position reports to the Director.
- Is constantly available by telephone/pager during assigned hours.
- Conducts urgent physical and psychosocial assessments of the patient’s and family’s after hours needs and determines interventions.
- Provides skilled nursing care focusing on pain control and symptom management.
- Provides interim guidance to the patient and his or her family regarding diagnosis, medications, treatments and the patient’s progress.
- Documents care in a thorough and timely manner.
- Works closely with members of the Interdisciplinary Team (IDG) to provide continuity of care and support for the patient and family.
- Collaborates with vendors to obtain DME, supplies and medications when needed after-hours.
- Makes arrangements for patient transfers to the hospital
- Collaborates with the hospital, SNF, and Residential Unit staff to adjust intervention and plan of care as needed.
- Conducts hospice admissions as necessary.

Pay: $36.00 - $39.00 per hour
