JOB RESPONSIBILITIES
- Provides safe and effective care for the client based on individualized Plan of Care.
- Provides prompt communication of changes in patient medical status/emergencies to the team resulting in effective management of cases.
- Demonstrate empathy, caring and self-confidence; ability to pay attention to details resulting in thorough follow through; anticipate problems in advance and knows whether to diffuse or escalate; prioritize and demonstrate a sense of urgency.
REQUIRED EXPERIENCE & EDUCATION
- Licensure: RN license or LPN license in the state of Georgia.
- Home care experience preferred.
- Excellent communication skills; team player; interpersonal skills; good collaboration skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
