Overview:
We are seeking a dedicated and compassionate Wound Care Nurse to join our healthcare team. The ideal candidate will have extensive experience in providing high-quality wound care to patients in a variety of settings, including nursing homes and workers' compensation environments. Our Wound Care Nurse will be responsible for delivering resident-centered care, promoting infection control, and ensuring effective communication with multidisciplinary teams. Duties:
- Conduct thorough wound assessments and develop personalized treatment plans for residents with chronic or acute wounds and prevention methods.
-Conduct weekly skin assessments on each resident
- Provide skilled nursing care to residents including administering medications and treatments.
- Collaborate with physicians, occupational therapists, and other healthcare professionals to ensure comprehensive care.
- Implement evidence-based wound care practices
- Monitor patient progress and adjust treatment plans as needed to achieve optimal wound healing outcomes as well as preventative methods
- Educate residents and their families on wound care techniques, infection control measures, and self-care strategies.
- Maintain accurate and detailed records of resident care, including wound assessments, treatment plans, and progress notes.
- Participate in quality improvement initiatives to enhance resident outcomes and reduce readmissions.
Requirements:
- Current license to practice as a Registered Nurse in the state where the position is located.
- Associate's or Bachelor's degree in nursing or a related field.
- Minimum of 2 years of experience with a focus on wound care.
- Experience working in a nursing home.
- Strong knowledge of wound care principles, including infection control.
- Certification in wound care is required.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment and prioritize tasks to meet resident needs.
