SNF and Clinic Coverage Needed in Vermont
Locum needed starting as soon as credentialed. Responsibilities include :
- Preventative visits, acute needs, and chronic ongoing needs.
- SNF admissions, discharges, acute needs, and periodic assessments.
- 1.5-2 days in clinic seeing patients aged 65 and up, and 3-3.5 days in the skilled Nursing Home.
- No procedures required.
- MA support and RN triage support along with MDs and APPs on the team.
- 8-12 patients per day at SNF and up to 16 patients in 8 hours in clinic.
- SNF uses PCC and EPIC; clinic is EPIC only.
