Description
Service Management Group (SMG), Inc. is a spinoff of Accra. We are a non-profit dedicated to improving lives through innovative home-based services and solutions. The Program Support Coordinator will support our Home Health program.
The Program Support Coordinator will interact with clients through phone calls, video calls, text messages, and emails to ensure visits and appointments are scheduled and the client's expectations are being met with a high level of customer satisfaction. The Program Support Coordinator will collaborate and coordinate with care managers, nurse consultants, clients, caregivers, providers, county care managers, technology / IT specialists, and other stake holders. The Program Support Coordinator is responsible to monitor and coordinate all incoming program referrals. Performs administrative duties to maintain and provide support for optimal home care program operations.
