What are the responsibilities and job description for the Self-Direction Liaison (SDL) a/k/a Self-Direction Coordinator position at ACDS, Inc.?
ACDS, Inc., a respected non-profit agency, is seeking a full-time Self-Direction Liaison a/k/a Self-Direction Coordinator for its growing Fiscal Intermediary (FI) Department in Nassau County, Long Island, NY. A minimum of 1 year of experience working with Intellectual or Developmental Disabilities (IDD) population is required.
Responsibilities for the Self-Direction Liaison a/k/a Self-Direction Coordinator include:
· Ensure compliance with agency, state and federal policies, procedures and regulations.
· Track and process all Medicaid compliance documentation.
· Prepare statistical data.
· Monitor individual budget utilization.
· Participate in Life Plan meetings, Circle of Support (COS) meetings and other person-centered planning meetings as necessary.
· Act as liaison between all members of the COS and Agency personnel
Qualifications for the Self-Direction Liaison a/k/a Self-Direction Coordinator role:
· 2 year college degree or higher preferred.
· Excellent written and verbal communication skills.
· Strong computer skills.
· Excellent phone manner.
· Team player.
