What are the responsibilities and job description for the Assessment Nurse position at Ace Care Management?
Position Summary
The Assessment Nurse is responsible for identifying current and future patient care needs. Being able to recognize normal versus abnormal body physiology. The roles require critical thinking, attention to detail and being able to prioritize appropriate interventions. An essential bridge between the assessment coordinator, inter-departments and external stakeholders that include, but is not limited to the Department of Community Health, health care providers, and medical institutions.
This role performs frequent analysis, excellent tracking skills, organizational skills, analytical and exceptional communication skills. Must always be committed to providing responsive aid in a courteous and professional manner.
Primary Duties and Responsibilities:
- Perform different aspects of the nursing process and function within the scoop of safe nursing practices.
- Work in supervisory role as warranted by Ace Care and its needs at the time appointed.
- Perform case management services for medically frail and compromised patients using best Case Management practices.
- Provide full MDS assessment for new and renewed members.
- Responsible for assisting with review of member assessments, implementations, and management of nursing services within the ACE Care scope.
- Provide disease management initiation and updates as needed

- Responsible for assisting with the initiation, development and maintaining plan of care.
- Assist in Identifying family/ client needs and for providing supportive care in accordance to the plan of care.
- Perform education to maintain standard of care within the nursing and case management department.
- Maintain regular communication with Administrator, other RN and LPN staff and lend assistance concerning client’s progress and problems.

- Must be available via phone or in person to resolve medical emergent situations.
- Follow up with compliance for physician’s orders and treatments being provided by direct care providers
- Communicate as needed with client’s physician, family members and client for any change in plan of care or services.
- Assist with Q.I program review as needed.
- Assist with provider meetings as needed
- Provide support and participate in IDT weekly meetings
- Represent the assessment department at the WAM (Weekly Ace meeting)
- Accept responsibility for coordinating physical care of the client, or providing direct care as needed.
- Responsible for knowing and understanding policies and procedures that governs the EDWP formally the SOURCE program.
- Must function independently in the absence of the administrator to maintain operation short term.
- Accepts other assignments as appropriate.

Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers and filing cabinets.
Position Type/Expected Hours of Work
This is a full-time position. Days and hours of work are Monday through Friday, 8:00a.m. to 5:00p.m.
