Position Summary 

 

The Assessment Specialist is responsible reviewing and analyzing all Assessments to ensure accuracy of data. Being able to recognize conflicting data and correct data in an efficient and precise manner. The role requires critical thinking, attention to details and being able to prioritize appropriate interventions. The role performs frequent analysis, requires excellent investigative skills, organizational skills, analytical and exceptional communication skills, both written and verbal. 

In addition, must possess leadership skills, ability to teach and train new staff on the assessment process. Ensure the assessment department functions in a seamless and productive manner as it relates to assessments. 

Collaborate with all departments to garner critical data to aid the assessment process.   

Primary Duties and Responsibilities: 

  • Perform different aspects of the assessment process review and function within the scope of safe assessment practices. 
  • Perform data management for medically frail and compromised patients using best assessment and investigative practices. 
  • Review the completion of different areas of the MDS assessment tool for new and reassessed members. 
  • Responsible for reviewing medical records and other health care documents to gather information that would be relevant to the assessment of our members. 
  • Review the submission of medical and non-medical data into ACMS to help qualify the need for member services. 
  • Provide assistance as part of the team for disease management. 
  • Provide updates as needed for the disease management in house process which is guided by a team approach. 
  • Analyze information gathered through the investigative process to ensure member’s assessment is accurate and comprehensive. 
  • Maintain regular communication with Administrator, nursing staff and lend assistance concerning client’s progress and problems. 
  • Maintain close communication with the assigned assessment nurses to ensure thoroughness and completion of the assessment. 
  • Communicate as needed with client’s physician, family members and client for any change in plan of care or services during the investigative process. 
  • Assist members in understanding their services and ensure correct and adequate service is maintained. 
  • Responsible for knowing and understanding policies and procedures that governs the EDWP formally the SOURCE program. 
  • Accepts other assignments as appropriate.

Work Environment 

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers and filing cabinets. 

Position Type/Expected Hours of Work 

This is a full-time position. Days and hours of work are Monday through Friday, 8:00a.m. to 5:00p.m.   

Travel 

No travel is expected for this position. 

Required Education and Experience 

  1. Bachelorsdegree/ Will accept Associates Degree for the right candidate.  
  2. At least two (2) years in a related field

Preferred Education and Experience 

  1. Bachelor’sDegree In Related Healthcare services 
  2. At least three (3) years in a related filed
