ACE Home Health Care & Hospice is seeking a per diem Medical Social Worker (MSW) for home health care in Contra Costa County.

Requirements : The ideal candidate must be a graduate of an accredited school of social work. At least 1 year of experience working in social work, in a home health care setting. Must have effective communication skills, knowledge of current community services available, be reliable and have a positive attitude, which fosters a cooperative and positive atmosphere.
Knowledge : Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations; must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify when to consult the intake supervisor. Must have knowledge of current Medicare, Medi-Cal, State and Federal regulations.
Job Types : Per Diem, Contract
Pay Grade : Estimated Annual Compensation : $140K to $202K (position is paid on a per-visit basis)
Salary : $140,000 - $202,000
