Intake/ Therapy Coordinator

Job Responsibilities:

• Evaluates patients referred, identifies insurance coverage, and coordinates referrals for services.
• Record all intake/therapy information and inputs into computer system and gets approval from DCS to proceed with referral/intake process.
• Works with physicians, nurses, therapists and discharge planners to establish a coordinated plan of care.
• Coordinates patient care with Therapists and process all Therapy related documents.
• Coordinate initial staffing of referrals and ensure receipt of referral for care by all disciplines ordered.
• Obtains and documents prior authorization for services from insurance providers.
• Obtain and submits documentation within required timeframes.
• Maintain strict confidentiality on patient, agency, and employee matters.
• Ability to demonstrate compassion and courtesy when dealing with difficult situations.
• Capable of working under pressure and time constraints including overtime to complete daily assessments.
• Ability to use sound judgment and tactful approach when dealing with co-workers, staff and the public.
• Perform other tasks as assigned.

Qualifications

Required Skills

• Minimum of one year of experience in intake/therapy coordinating.
• Self-starter who enjoys working in a fast-paced environment with the ability to multi-task.
• Excellent problem-solving, observation, written and verbal communication skills.
• Knowledge of overall home health management including admission criteria, nursing process and insurance limitations.
• Basic knowledge of insurances including Medicare and Medicaid.
• Effective interpersonal communication skills.
• Fluent knowledge of English, both verbal and written.'

Pay: $24.00 - $26.00 per hour
