Job Responsibilities:
- Evaluates patients referred, identifies insurance coverage, and coordinates referrals for services.
- Record all intake/therapy information and inputs into computer system and gets approval from DCS to proceed with referral/intake process.
- Works with physicians, nurses, therapists and discharge planners to establish a coordinated plan of care.
- Coordinates patient care with Therapists and process all Therapy related documents.
- Coordinate initial staffing of referrals and ensure receipt of referral for care by all disciplines ordered.
- Obtains and documents prior authorization for services from insurance providers.
- Obtain and submits documentation within required timeframes.
- Maintain strict confidentiality on patient, agency, and employee matters.
- Ability to demonstrate compassion and courtesy when dealing with difficult situations.
- Capable of working under pressure and time constraints including overtime to complete daily assessments.
- Ability to use sound judgment and tactful approach when dealing with co-workers, staff and the public.
- Perform other tasks as assigned.
Qualifications
Required Skills
- Minimum of one year of experience in intake/therapy coordinating.
- Self-starter who enjoys working in a fast-paced environment with the ability to multi-task.
- Excellent problem-solving, observation, written and verbal communication skills.
- Knowledge of overall home health management including admission criteria, nursing process and insurance limitations.
- Basic knowledge of insurances including Medicare and Medicaid.
- Effective interpersonal communication skills.
- Fluent knowledge of English, both verbal and written.'
