If You Want Flexibility and To Make A Difference In The Community, Join Acenda's Integrated Health Care Team As A Case Manager for Our Integrated Case Management Services.
Key Responsibilities:
- Engages consumers at the point of hospitalization and participates in the discharge planning process to assist with the transition to the community
- Provides post-discharge follow-up to support the consumer with community integration
- Conducts assessments within correctional facilities and attends court hearings to advocate for persons served.
Requirements:
- Minimum requirement of a Bachelors Degree in Human Services or Psychology.
- At least 2 years of experience in mental services preferred
- Professionalism and a commitment to excellence in care
- Must have and maintain a valid driver's license with an acceptable driving record.
