Program Overview :
The Adult Partial Care (APC) program assists individuals who have severe and persistent mental illness with developing the skills and support necessary to live, learn and work with the least amount of intervention from the behavioral health system. The Adult Partial Care program offers comprehensive, individualized and quality behavioral health treatment within a group setting, designed to promote maximum independence in the least restrictive setting.
Key Responsibilities :
- The counselor plans and provides group services around consumers IRP treatment needs. Services include engagement strategies, activities to meet meaningful goals over time, Illness Management and Recovery Program, skill development, prevocational services, counseling, learning activities, social / leisure services; as well as offsite interventions.
- In addition to individual and group support services, counselors are responsible for maintaining authorization timeframes, intake and daily documentation, and service planning.
- Demonstrates competent written and verbal communication with referral sources and external customers.
- Attentive and responsive to customer needs in a helpful, supportive and timely manner.
- Works collaboratively with coworkers and agency staff to effectively communicate and problem-solve.
- Meets obligations of the program (contractual, licensing, Medicaid, etc.).
- Follows program policies and procedures and encourages the same in others.
- Educates self on issues impacting the program population.
- Operates conscientiously in accordance with program budget (i.e., use of client engaging funds, office supplies).
