Program Overview: LifeSet is an individualized, evidence-based community-based program that is highly intensive. LifeSet specialists meet with participants face to face at least once each week. They text, email and call young people regularly throughout the week, when needed. Specialists stabilize even the toughest situations and help young people build healthy relationships, obtain safe housing, education and employment. LifeSet is one of the nation’s first — and now one of the largest — evidence-based programs helping young people who age out of foster care.
Position Summary: The Specialist is responsible for providing services to address the needs of young people in their day-to-day lives. The LifeSet Program is an evidence-informed program model to address the needs of young people who are transitioning out of foster care and need individualized skill development and support to transition successfully into adulthood. The role will include, but not limited to facilitating the following processes for program participants in obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system, and remaining free from legal involvement
Key Responsibilities:
- Meet one on one with youth in the community.
- Work collaboratively with the youth and their support system to develop a comprehensive monthly treatment plan.
- Be available to the youth and the program staff regularly.
- Be available for all regularly scheduled meetings.
- Be open to constructive criticism.
- Have a flexible work schedule, with on call rotation.
- Have a strong understanding of community resources.
- Be open to driving for long distances.
- Have strong client engagement.
Requirements:
- Master’s level or Bachelor's level practitioner with 2 or more years of experience providing direct care to youth in a behavioral health agency or institutional setting.
- Required to work flexible hours, and be on call 24/7 (365 days), when required by the program.
- Must have and maintain a valid driver's license with an acceptable driving record
- Must be able to exercise independent thinking and good judgment under all circumstances.
- Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.
Additional Information:
- Hourly Rate: $24.04
- Bilingual Rate: An additional $1.50/hourly incentive added if Spanish proficient
