Overview
We are seeking a dedicated and experienced Director of Case Management to lead our team in providing exceptional patient care. Duties
- Lead and manage the case management team to ensure effective patient care coordination.
- Develop and implement policies and procedures that meet organizational standards and regulatory requirements.
- Conduct utilization reviews to assess the appropriateness of services provided to patients.
- Collaborate with multidisciplinary teams to create individualized care plans for patients, including those in acute care and hospice settings.
- Provide guidance on addiction counseling strategies and behavioral therapy interventions.
- Monitor patient progress and adjust care plans as necessary to achieve optimal outcomes.
- Ensure compliance with all relevant healthcare regulations and standards.
- Facilitate training and professional development opportunities for case management staff.
Experience
- Large Acute Carerequired.
- Strong background in behavioral therapy and addiction counseling preferred.
- Proficient understanding of medical terminology and documentation standards.
- Excellent communication skills, both verbal and written, with the ability to interact effectively with patients, families, and healthcare professionals.
- Proven leadership experience with a track record of developing high-performing teams.

Pay: $110,488.00 - $128,720.00 per year
