Manager of Case Management
Job Overview
The Manager for Case Management services reports to the Case Management Director and is responsible for promoting patient-centered care by coordinating all aspects of hospital-based case management for his/her assigned area of responsibility in alignment with the goals of the Case Management Department.
This role is accountable for the overall day-to-day oversight and management of the case management program including coordination, supervision, and administrative oversight of the case management team. This role will function as an expert clinical practitioner, case management subject-matter expert, resource, advisor and leader for the members of the case management team. Supervises and monitors professional and support staff and ensures that effective care coordination and case management practices are consistent with hospital policies, and applicable regulations and guidelines. Monitors quality of the interactions, documentation and assessments of the case management staff. The Manager for Case Management Services will coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization.
Duties
- Assess patients' needs and develop individualized care plans based on their medical history and current health status.
- Coordinate with multidisciplinary teams to implement care plans effectively.
- Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
- Provide education and support to patients and families regarding treatment options, self-management strategies, and available resources.
- Facilitate discharge planning for patients transitioning from inpatient to outpatient care.
- Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with regulatory standards.
- Advocate for patients’ needs within the healthcare system, ensuring access to appropriate services.
- Collaborate with providers in various specialties including Pediatrics, Primary Care, Medical-Surgical, Infectious Disease Care, Behavioral Health, and Managed Care.
Experience
- Current Registered Nurse (RN) license in the state of practice.
- Previous experience in case management or a related nursing field is preferred.
- 3 years’ experience in Acute-Care Case Management or Social Work
- 2 years supervisory experience
- Ability to work independently while managing multiple cases efficiently.
- Knowledge of community resources and support services available for patients.

Pay: $80,000.00 - $120,000.00 per year
