Time Type: Full time Department: CC014253 Specialty Pharmacy Summary: M-F 8a-4:30p - Specialty Pharmacy The Pharmacy Coordinator provides clinical and educational services to patients and other healthcare providers in a specific practice area and acts as a resource for staff pharmacists with problems related to patient medication therapy. This person also functions as a liaison with medical and nursing staff on medication related issues and assists other department managers in optimizing overall provision of service to patients. Additional Information: #AC2024 Required Education: Doctorate of Pharmacy or Bachelor of Pharmacy (if obtained prior to 2000) Recommended Education: Doctorate Required Work Experience: Pharmacy - 2 years of experience or Residency trained Recommended Work Experience: Required Certifications: Pharmacist - Arkansas State Board of Pharmacy Recommended Certifications: Board Certified Pharmacotherapy Specialist (BCPS) - Board of Pharmacy Specialities Description 1. Works with Director and others to plan, implement, analyze, monitor and maintain a safe and effective medication use process. 2. Develops and maintains competency with hospital computer system to enable identification of safety issues and assist other health care professionals in safe use of the system. 3. Uses Safety Tracker to report interventions and medication occurrences. Uses other documentation systems as requested. 4. Provides clinical and educational services such as rounding, drug information, patient education and optimization of drug therapy to patients and other health care professionals in selected practice site. 5. Serves as consultant to pharmacists and department in areas of clinical service. Assists pharmacists in resolving problems related to physician orders, therapy, medication preparation and P&T Committee requirements. 6. Acts as a liaison between Pharmacy and medical and nursing staff on issues pertaining to clinical services or other pharmacy issues. 7. Educates pharmacists and other healthcare providers in areas of pediatric drug therapy through in-services, presentations, written educational ,materials and other methods. Two CE programs a year for pharmacists are required. 8. Participates in training of pharmacy students and residents. Acts as preceptor as needed or requested. 9. Substitutes as staff pharmacist and assists with distributive workload as requested by managers. 10. Assists in analysis, planning and implementation of new or revised services. 11. Performs other duties as assigned.
