Position type:

• Part-time, hourly shifts (Sunday 10:00a-9:00p, Monday 3:00p-9:00p & Thursday 3:00p-9:00p)

Salary/rate:

$16.08 per hour

Location: Bridgeville, PA

Travel: Must be willing to provide transportation using your own personal vehicle. Mileage reimbursement provided.

Position Description

Provides direct support for people in their home and/or community. Ensures the health, safety, and well-being by assisting in acquiring, maintaining and improving self-help, socialization, and/or community living skills. Ensures activities are centered around wants, interests and needs and adheres to established goals and outcomes chosen by each person and their support team.

Job Responsibilities - Highlights

• Demonstrates a person-centered practice's approach by working on each person's goals and outcomes.
• Maintain a healthy balance between supporting a person's health and safety and their self-determination.
• Completes required trainings.
• Assists each person in maintaining a healthy and safe environment.
• Communicates with supervisor and participates in staff meetings and supervision.
• Maintains a professional and cooperative working relationship with families/other team members.
• Demonstrates and understands the responsibilities of reporting incidents per state and federal guidelines.

Qualifications

Minimum requirements:

• Equivalent of a high school diploma or GED with one year of related experience as a provider of direct care support for persons with disabilities.
• Valid driver's license and properly insured vehicle.
• Satisfactory ACT 33/34 as well as FBI clearances required.
• Must have a smart device.
• Must obtain First Aid/CPR certification within six months of start date.
