Job Summary:
The Adult Care Manager is responsible for coordinating and managing care services for adults, particularly those with chronic illnesses, disabilities, or mental health needs. This role involves assessing client needs, developing care plans, connecting clients with appropriate services, working closely with the entire healthcare and social services team and advocating on their behalf to ensure they receive high-quality, person-centered care.
Key Responsibilities:
• Conduct comprehensive assessments of clients’ medical, psychological, and social needs.
• Develop and implement individualized care plans based on client needs and goals.
• Coordinate services such as healthcare, housing, transportation, and social support.
• Serve as a liaison between clients, families, healthcare providers, and social services.
• Monitor and evaluate client progress, adjusting care plans as needed.
• Provide crisis intervention and support in urgent situations.
• Advocate for clients’ rights and access to necessary services.
• Maintain accurate and up-to-date documentation
• Educate clients and families on available resources, benefits, and self-care strategies.
• Ensure compliance with all NYS-DOH relevant laws, policies, and ethical guidelines.
Qualifications:
Bachelor’s degree preferred. Experience in the following is preferred but not mandatory: Care coordination, case management, social services, or healthcare. Experience working with adults with disabilities, mental health conditions, or chronic illnesses is preferred.
