PURPOSE: Provides linkage to care and non-medical case management services to individuals who are newly diagnosed with Human Immunodeficiency Virus (HIV). Using motivational interviewing techniques, provide education and counseling in a culturally competent manner regarding infection and importance of medical care and medication adherence. Locates infected individuals who have been lost to care, identifies barriers to attendance at medical appointments, and works to address barriers so client can be re-linked to medical care.
Participate in quality improvement activities. Maintain all files and complete program related data entry and reports. Work with clients to complete all recertification documents within established timeframes for submission to ensure uninterrupted client services.
DUTIES:
Non-Medical Case Management and Linkage to Care: Contact patients newly diagnosed with HIV and offer linkage to care and non-medical case management, according to the standards and guidelines set forth by Virginia Department of Health, HIV Care Services, and Human Resources and Services Administration (HRSA). Motivate patients to attend medical appointments and remain adherent to their medication regimen.
Through the use of motivational interviewing techniques, identify barriers that may prevent the patient from attending appointments. Link patients to additional services to reduce the barriers identified.
- Determine eligibility for patients newly diagnosed with HIV. Assist 95% of patients with scheduling a first medical appointment within 3 days of determining eligibility for services. Ensure that 90% of clients attend their first medical appointment within 30 days of enrolling in linkage to care or non-medical case management services. Provide non-medical case management to RW clients. Working with the client, complete and submit recertification documents for non-medical case management clients according to Virginia Department of Health (VDH) standards and within specified time frames to ensure no interruption in client services. Assist with enrollment in qualifying health plans when appropriate and within required timeframes. Demonstrate proper motivational interviewing techniques through role play or observation by supervisor at least quarterly. Demonstrate ability to present factually accurate information in a manner appropriate for the intended audience through role play or observation by supervisor at least quarterly.
Follow Up on Patients Who Are Lost to Care: Initiate investigative activity on all lost-to-care clients within 2 business days of assignment. Perform investigative activity on all lost-to-care clients at least 3 times/week until closed. Close all lost-to-care clients within 30 days of assignment. Follow protocols provided by VDH for lost- to-care follow up. Document barriers to care and efforts to reduce barriers. Complete enrollment for non- medical case management or patient navigation services for clients who are located and re-linked. All documentation meets VDH standard and demonstrates that established VDH protocols were followed.

File Maintenance and Data Entry: Maintains all documents required by VDH for case files. Enters all program related data into data base.
- 95% of all case files contain all required documents with necessary signatures. Case files are organized and legible. Data entry is completed within 2 days of enrollment into program, and updated within 2 days of follow-up activity. Data entry is accurate 95% of the time. Progress notes are legible, factually accurate, and complete.
Training and Professional Development: Attends continuing education programs in accordance with development plan. Attends other job related trainings and programs. Identifies own learning and training needs related to enhancing job performance or maintenance of professional skills. Investigates appropriate trainings, and seeks appropriate permission to attend. Uses information gained at trainings to improve personal skill level and shares information with co-workers and team members. Makes noteworthy contributions to improving processes and service delivery.
- Maintains TRAIN account. Completes required new employee training within established timeframes.
Completes required updates as requested and within requested timeframes. Identifies and participates in 2-4 additional HIV related webinars, trainings, or conferences/year with prior supervisory approval. Actively participates in team meetings and contributes to discussions regarding improvement of client services.

High school (HS) diploma or General Education Development (GED) and one year of experience working with persons living with HIV and/or health care training for example certified medical assistance or medical clerk.
All Non-Medical Case Manager/Eligibility/Intake Specialists complete a minimum training regimen within one year of hire date that includes: (a) AIDS Drug Assistance Program (ADAP) requirements and application, (b) Medicaid, Medicare, Supplemental Security Income (SSI), Social Security Disability Insurance (SSDI), (c) VDH’s HIV Case Management standards (d) cultural competency, and (e) RW eligibility. If newly-hired Non-Medical Case Manager/Eligibility/Intake Specialists have previously obtained all of the required training, they do not need to repeat it. Documentation of completion of required trainings must be kept in the Non-Medical Case Manager/Eligibility/Intake Specialist& personnel file.
Five hours of continuing education in HIV/AIDS is required annually. Ongoing training on changes to benefit program and their eligibility, such as Medicare, Medicaid, SSI, SSDI, RW etc. is also required annually. Documentation of completion of required trainings must be kept in the Non-Medical Case Manager/Eligibility/Intake Specialist's personnel file.
A valid State Driver’s License is required to operate a state vehicle or if operating a personal vehicle while conducting business on behalf of the agency.
Employment is contingent upon satisfactory results of a state and federal criminal history background check and the Department of Social Service’s Child Abuse and Neglect Central Registry check, U.S. HHS IG Exclusion List check, employment reference check, proof of education (diploma or transcripts), professional licensure verification, and E-Verify. Conviction information, ascertained through a fingerprint-based background check, is a final step in the application process (if selected for the position).
Documentation of immunization status must be provided within 10 working days of employment.
