Job Title : Care Coordinator - full-time
Location : Glen Cove, NY (Travel throughout Nassau County required)
Hourly Rate : $26.50 per hour; 35 hours per week schedule
Schedule : Monday - Friday (some flexibility needed to meet caseload needs)
Position Overview :
The Charles Evans Center is seeking a compassionate and skilled Care Coordinator to join our expanding care coordination team. In this role, you will be a vital member of an interdisciplinary team dedicated to providing care coordination for adults diagnosed with severe mental illness, multiple medical co-morbidities, and / or co-occurring substance abuse disorders.
Key Responsibilities :
- Advocate for and support clients in navigating the healthcare system.
- Engage with community agencies and healthcare providers to ensure clients have access to necessary services.
- Facilitate wellness self-management and reduce emergency room visits and hospitalizations.
- Provide expert support and psychosocial / substance abuse interventions.
- Maintain a focus on client outcomes and best practices.
